Cancellation Policy

At Le Formulaire we are dedicated to treating our clients with the highest standard of care possible. We understand that life can sometimes interrupt our schedule, however if an appointment is cancelled at late notice it prevents another client from being seen.

At Le Formulaire, we operate with a 48 hour cancellation policy.

If an appointment is cancelled more than 48 hours prior to the scheduled time, any deposit paid will be refunded and a new appointment can be rescheduled at no additional cost.

If a cancellation is made within 48 hours of the appointment for any reason, Le Formulaire will retain the deposit. If you arrive very late for an appointment, we may require you to reschedule and reserve the right to follow the policy described above. If you do not attend an appointment without notice you may be charged for the value of the treatment scheduled. 

We reserve the right to charge any card you hold on file with Le Formulaire in accordance with the above policy.
